Acute GP service

The Admiral Nurse role works with the safeguarding team to provide support for complex care needs and carers. If someone is caring for a loved one at home, with dementia or any complex needs, we can give them Caroline's number to call, should they want or need to. Caroline will help support the patient and the carers. There's various schemes, link ups, cafe's and carer support groups associated with this. So please feel free to share this number with any patient or relative who may be interested now or in the future.
